Our duties
Dixon Behavioral Health is required by law to maintain the privacy of your Protected Health Information (PHI), provide you with this notice, and abide by the terms of the notice currently in effect.
How we use and disclose your PHI
Treatment: We use your PHI to provide and coordinate your behavioral health care.
Payment: We use your PHI to bill your insurance and collect payment for services.
Healthcare operations: We use your PHI for quality improvement, training, and program operations.
Your rights
You have the right to: inspect and copy your records; request corrections; request restrictions on disclosure; receive confidential communications; receive an accounting of disclosures; and file a complaint with us or with the U.S. Department of Health and Human Services.
